Due to technical difficulties with the claim form, the deadline to submit a claim in the Eldercare Lawsuit settlement has been extended to Friday, July 6, 2018, at 11:59 p.m. If you would like to submit the claim form on-line and experience technical difficulties, please contact the Claims Administrator, Gary Goodstein, at 213-683-1908, beginning on Monday, July 2nd. Mr. Goodstein will walk you through the on-line submission process at that time. Thank you for your patience.
The following is an email was sent by Gary J. Goodstein (Fallbrook/Erwin Eldercare Development Settlement Fund Claim Administrator) on Saturday 6-30-18:
While most claimants have had no difficulty submitting their claims, I have received a few emails this morning from individuals who have had technical issues when they attempted to submit a claim electronically.
In order to address these technical issues without prejudicing or unduly inconveniencing claimants or the Claim Administrator, THE CLAIM SUBMISSION DEADLINE IS EXTENDED TO FRIDAY, JULY 6, 2018.
SUGGESTIONS FOR SUBMITTING CLAIM FORM:
- The form, including the “SUBMIT” button at the end, should work without problem if you use Adobe Reader to open and complete it. Adobe Reader can be downloaded for free athttps://get.adobe.com/reader/
- If you use the Microsoft Internet Explorer browser, the form, including the “SUBMIT” button at the end, should work without problem.
- If you use the Microsoft Edge browser, the form can be completed online but can only be submitted if you right-click on the form, select “Save As” to download the form as a pdf file, and then email the saved file either directly or by opening the file in a pdf program such as Adobe Reader and using the “SUBMIT” button at the end of the form.
- If you use the Google Chrome browser, the form contents do not save if you fill them out online and the “SUBMIT” button does not function. Instead, download the form and then use pdf software such as Adobe Reader to complete and submit it.
- If you use the Firefox browser, the form appears in read-only format. Download the form and then use pdf software such as Adobe Reader to complete and submit it.
- If you use the Safari browser, the form appears in read-only format. Download the form and then use pdf software such as Adobe Reader to complete and submit it.
- If you use the Opera browser, the form contents do not save if you fill them out online and the “SUBMIT” button does not function. Instead, download the form and then use pdf software such as Adobe Reader to complete and submit it.
THERE ARE SEVERAL WAYS THE FORM CAN BE SUBMITTED. IT IS NOT NECESSARY TO SUBMIT YOUR RESPONSES ON THE FORM ITSELF:
- Electronically, by using the “SUBMIT” button at the end of the Claim Form;
- Electronically, by attaching the completed Claim Form to an email addressed to Cl***@Go****************.com;
- Electronically, by typing your answers to the Claim Form on a separate text document (such as a Word or Wordperfect) and attaching the completed document to an email addressed toCl***@Go****************.com;
- Electronically, by typing your answers to the Claim Form directly into the body of an email addressed to Cl***@Go****************.com;
- My mail, by printing out the completed Claim Form and placing in an postage-paid envelope addressed to Claim Administrator, 11355 W. Olympic Blvd., Suite 300, Los Angeles, CA 90064 (Please note that if you print the form, only the visible text in each box will appear. If your answer to a given question is longer than about 3 lines, please type your responses onto a separate piece of paper instead so your entire response will be viewable).
CONFIRMATION OF RECEIPT:
I have endeavored to send an email acknowledging receipt of each Claim Form I received that included an email address. If you submitted a claim that included an email address and have not received a confirming email, please either resubmit your claim or contact my office to confirm receipt.

The Claim Administrator operates independently of WANA and/or the PRESERVE WALNUT ACRES neighborhood associations, and is the only reliable source of information or assistance regarding the claim process. The Claim Administrator will not communicate about individual claim issues through proxies or third-party advocates, including WANA or PRESERVE WALNUT ACRES.
